Puppy Questionnaire & Preference Form

Bringing a Labrador Retriever pup into your home is a lifetime commitment.

We are looking for those who understand or have a willingness to learn the needs of a Lab and therefore ask you to answer as honestly and thoroughly as you can. We hope to better understand your preferences and match you with a puppy that fits your specific needs and desires.

This form does not commit you to a puppy or lock you into a reservation, nor does it commit us to place a puppy in your home; it is a first step, telling us what you want in a dog, so we can recommend to you which puppy we believe fits your needs as well as helping us find our beloved puppies the most appropriate and best home possible.

After you've filled in the application, we will respond within two (2) business days. Once we have reached out to you to discuss any questions and/or concerns, you will receive approval and you can join our Reservation List by submitting a $600 NON-REFUNDABLE Reservation Fee, which will come off the total price of the puppy. The puppies are sold with a limited registration and a 2-year health guarantee*. They will be examined by our licensed veterinarian, dewormed, first set of puppy vaccines, exposure to the crate & potty training, raised with an Empowered Puppy curriculum, handled daily, raised in our home and you will received a lifetime and beyond judgment-free support by us. Our puppy price starting for all 2025 litters is $2,000.

Properly socializing your puppy is the key to raising a healthy, well adjusted dog. Please describe how you will do this.

Do you have any concerns about getting a puppy?

SPAY/NEUTER... Recent research has shown benefit to waiting to spay or neuter your puppy until at least 2 years of age or older. Would you be willing to consider this? Are you planning on leaving the dog intact?

GOTCHA DAY. How are you planning to pick up your Puppy from us in Massachusetts? If you are from out of state, Providence (PVD) and Boston (BOS) are the closest national airports. Or we can deliver to you for an additional cost.

Requesting a reservation & sending a reservation fee once approved or only desiring feedback regarding your needs? Reservation fees are accepted after your application is approved. Your reservation fee will place you on our reservation selection list.
